Diabetes is a complex disease that can be challenging to manage on your own. Diabetes self-management training can help you manage your diabetes and live a healthier life.
Our outpatient program provides diabetes self-management training taught by certified diabetes care and education specialists and registered dietitians.
Our program is recognized by the American Diabetes Association for meeting national diabetes education standards.
All diabetes self-management training requires a physician referral. As a courtesy, we will verify your insurance coverage prior to your first appointment. We may ask you to check your coverage if your insurance coverage will not provide the information to us. Medicare, Medicaid and many private insurance companies cover for diabetes self-management training.
Individual appointments and classes are offered at Wood County Hospital. In addition, individual appointments are also available at Falcon Health, Bowling Green Clinic, Bowling Green Family Care Center, Deshler Family Practice and North Baltimore Family Practice. Individual Diabetes Assessment (approximately 1 hour)
Class Series - Morning and Evening sessions are offered in a 2 or 3 class series for a total of 6 hours of teaching
